A Primobolan cycle is a course of methenolone, a mild DHT-derived anabolic steroid used mainly during cutting phases to hold on to muscle while losing fat. In bodybuilding circles, a typical Primobolan cycle is reported as 8 to 12 weeks of the injectable form at 400 to 800mg per week, almost always alongside testosterone. Methenolone does not convert to estrogen, so it causes no water retention or estrogen-driven gynecomastia. But it still shuts down natural testosterone, it worsens cholesterol, and the oral form has caused serious liver injury in clinical use.

This guide covers what Primobolan is, how people actually run it, what results are realistic, and what the medical literature says about its risks. It also covers what the research says about post-cycle recovery, which is less reassuring than most cycle guides suggest. Nothing here is a recommendation to use it. Methenolone is a controlled substance in the US and many other countries, and most of what is sold under its name on the black market is not what the label says.

Primobolan (methenolone) is a controlled substance in the United States and many other countries. This article is for educational and harm-reduction purposes and is not medical or legal advice. Anabolic steroids carry serious health risks, including heart disease, liver injury, hormonal disruption and infertility. Speak with a qualified healthcare professional before making any decision about anabolic steroids. Primobolan is prohibited by WADA in all competitive sports. Muscle & Brawn does not encourage the use of anabolic steroids or of prescription drugs obtained outside a legitimate medical relationship.

Key Takeaways

  • Primobolan (methenolone) has a commonly cited anabolic rating of 88 and androgenic rating of 44-57, making it far weaker than testosterone. It preserves muscle in a calorie deficit but builds little new mass.
  • Commonly reported men’s doses are 400-800mg per week injectable, run with testosterone. These figures come from user practice, not clinical trials.
  • In a clinical study of 28 women given methenolone enanthate, 12 developed high cholesterol disorders and one had a heart attack during treatment.
  • In a 2023 study of 641 men who had stopped anabolic steroids, only 48% had fully recovered their hormone levels. Stacking more compounds lowered the odds of recovery.
  • A 2022 meta-analysis found 36% of black-market steroids were counterfeit and a further 37% substandard. Expensive compounds like Primobolan are prime targets.

What Is Primobolan?

Primobolan is the original brand name for methenolone (also spelled metenolone), an anabolic steroid derived from dihydrotestosterone (DHT). Schering developed it in the early 1960s. Doctors used it for decades to treat aplastic anemia and other conditions where patients were losing weight and muscle. Methenolone acetate tablets are still used for anemia in Japan, and a 2024 case report documents that use in an older patient with myelodysplastic syndrome.

It comes in two forms:

  • Methenolone enanthate (injectable): The form most bodybuilders mean by “Primo.” Its commonly cited half-life is about 10 days, so it is usually injected once or twice a week.
  • Methenolone acetate (oral): Tablets with a half-life of only a few hours, taken daily. It is less bioavailable, so reported oral doses run higher per week than injectable ones.

Methenolone’s commonly cited anabolic rating is 88 and its androgenic rating 44-57. Testosterone scores 100 on both. That low androgenic score is the source of its reputation as a “mild” steroid, and also why it is rarely used to bulk.

How Primobolan Works

Methenolone binds to the androgen receptor like other anabolic steroids. In muscle, that raises protein synthesis and helps the body hold on to nitrogen, which matters most when calories are low and the body would otherwise break muscle down for fuel. This is the main reason people run a Primobolan cycle during a cut.

Because methenolone is already a DHT derivative, it is not converted to estrogen by the aromatase enzyme. It is also not a substrate for 5-alpha reductase, the enzyme that turns testosterone into DHT. Both facts shape its side effect profile, covered below.

Primobolan Cycle Protocols: What Users Report

The tables below describe how Primobolan cycles are commonly run, based on user reports and coaching literature. They are not clinical doses and they are not recommendations. No controlled study has tested methenolone at these doses in healthy men.

Almost every reported Primobolan cycle includes testosterone. Methenolone suppresses the body’s own testosterone production, and its own androgenic effect is too weak to replace it. Men who run it alone tend to report low-testosterone symptoms: low libido, fatigue and low mood.

Commonly Reported Entry-Level Cycle (10-12 Weeks)

WeeksPrimobolan (Injectable)Testosterone Enanthate
1-12400mg/week300-500mg/week

This is the most commonly described “first” Primobolan cycle. The weekly dose is usually split into two injections.

Commonly Reported Intermediate Cycle (12 Weeks)

WeeksPrimobolan (Injectable)Testosterone Enanthate
1-12600mg/week150-200mg/week

At this level, users often cut testosterone to roughly replacement-level doses and raise the Primo. The goal is to limit estrogen-related side effects from testosterone, since Primo itself does not aromatize.

Commonly Reported Advanced Cycle (10-12 Weeks)

WeeksPrimobolan (Injectable)Testosterone EnanthateTrenbolone Acetate (optional)
1-10800-1000mg/week150-200mg/week200-300mg/week

Adding trenbolone changes the risk profile entirely. Trenbolone is far more androgenic, and each extra compound makes recovery harder (see the PCT section below).

Primobolan Dosage by Form

Primobolan is weak per milligram, which is why reported doses look high next to other steroids. It is also one of the most expensive steroids, so high-dose cycles are costly to run and a common target for counterfeiters.

Injectable Primobolan Dosage (Reported Ranges)

GroupWeekly DoseReported Cycle Length
Men, entry level400mg/week10-12 weeks
Men, intermediate600-700mg/week10-12 weeks
Men, advanced800-1000mg/week8-12 weeks
Women50-100mg/week6-8 weeks

Oral Primobolan Dosage (Reported Ranges)

GroupDaily DoseReported Cycle Length
Men, entry level50-75mg/day6-8 weeks
Men, intermediate100-150mg/day6-8 weeks
Men, advanced150-200mg/day6-8 weeks
Women25-50mg/day4-6 weeks

Oral methenolone acetate is not 17-alpha-alkylated, the chemical change that makes oral steroids like Anadrol and Winstrol hard on the liver. Its liver risk is therefore usually described as low. That is not the same as zero. In one documented clinical case, a 75-year-old aplastic anemia patient developed sharply raised liver enzymes after starting methenolone acetate. He went on to liver failure and died. The authors noted that liver injury from the drug had been thought “mild, reversible and rather infrequent,” and urged caution. The patient was elderly and very ill, but the case shows the risk is real.

Primobolan Results: What to Expect

Primobolan is not a mass builder. People who expect Dianabol-style weight gain from a Primobolan cycle are usually disappointed. Its results are subtle and show up mainly during a cut.

Realistic Primobolan Cycle Results

  • Muscle preservation: Its main use. In a calorie deficit, Primo helps hold on to lean tissue that would otherwise be lost.
  • Fat loss: Users report body fat coming down over an 8-12 week cycle. This depends on the diet. Primo is not a fat burner on its own.
  • Harder, drier look: With no estrogen-driven water retention, muscle looks more defined. This is why it is popular in contest prep.
  • Strength maintenance: Primo rarely adds much strength, but users report it limits the strength loss that comes with dieting.
  • Vascularity: Lower body fat and less water under the skin make veins more visible.

What Primobolan Will Not Do

  • Add large amounts of muscle (people looking for size usually turn to testosterone or nandrolone)
  • Produce fast, dramatic changes
  • Make up for poor diet or training

Most users report visible changes after 4-6 weeks, with the full effect by weeks 8-12. These timelines come from user reports, not trials.

Primobolan Side Effects

Primobolan’s side effects are milder than those of most anabolic steroids. Milder is not the same as safe, and some of its risks are better documented than cycle guides usually admit.

Cholesterol and Heart Risk

This is the best-documented risk. In a German clinical study, 28 postmenopausal women with breast cancer were given methenolone enanthate. Twelve developed hyperlipoproteinemia (abnormally high blood fats, mostly LDL cholesterol). One had a heart attack during treatment. The lipid changes went away in every case once the drug was stopped.

These were women receiving clinical doses, far below what bodybuilders report using. Higher doses, testosterone in the stack and repeated cycles are unlikely to make this better. Anabolic steroid use more broadly is linked to heart muscle damage, worse lipids and faster artery disease, according to a 2017 American Heart Association scientific statement.

Testosterone Suppression

Methenolone suppresses the body’s own testosterone production. It is often described as less suppressive than nandrolone or trenbolone, but there is little hard data comparing them. Suppression is the reason nearly every reported cycle includes testosterone and is followed by some form of post-cycle therapy.

Hair Loss

As a DHT derivative, methenolone can speed up male pattern hair loss in men who are genetically prone to it. Finasteride, often used for hair loss, works by blocking 5-alpha reductase. Methenolone does not go through that enzyme, so finasteride does nothing to reduce its effect on the scalp.

Virilization in Women

Women face a risk of masculinizing effects: a deeper voice, facial and body hair, clitoral enlargement and acne. Some of these, especially voice changes, can be permanent. See the section on women below.

Injection Site Reactions

Pain, redness or swelling where the injection was given. Non-sterile underground products raise the risk of infection and abscesses.

Side Effects Primobolan Does Not Cause

  • Estrogenic gynecomastia: Primo does not aromatize, which puts it among the steroids that don’t cause gyno on their own. Any gyno risk in a stack comes from the testosterone.
  • Estrogen-driven water retention or bloating

Primobolan PCT: What the Research Shows

Post-cycle therapy (PCT) is the practice of taking drugs such as tamoxifen (Nolvadex) or clomiphene (Clomid) after a cycle to restart the body’s own testosterone production. Neither drug is approved for this use.

What Users Typically Run

Because injectable methenolone enanthate stays in the body for weeks, users commonly report starting PCT about two weeks after the last injection. With the oral form, PCT is usually started within a few days. The most commonly reported protocols are:

PCT DrugCommonly Reported ProtocolDuration
Tamoxifen (Nolvadex)40mg/day weeks 1-2, 20mg/day weeks 3-44 weeks
Clomiphene (Clomid)50mg/day weeks 1-2, 25mg/day weeks 3-44 weeks

What the Evidence Says

The best recent data comes from a 2023 study of 641 men seen at a UK clinic after stopping anabolic steroids. Only 48% had fully recovered their reproductive hormones (LH, FSH and testosterone back in the normal range). Other findings:

  • PCT was linked to faster short-term recovery. Men who used PCT recovered in a median of 13 weeks, compared with 26 weeks for men who did not. That link only held for men who had stopped within the previous three months.
  • More compounds meant worse odds. Men who had used four different steroids were about a quarter as likely to recover as men who had used one.
  • The authors did not endorse PCT. They described it as “illicit, ill-defined, and not recommended,” and called for proper research.

For anyone thinking about a Primobolan cycle, the practical point is that a mild compound does not guarantee a full recovery. Every extra drug in the stack lowers the odds. Blood work before and after is the only way to know where recovery stands. Our PCT guide covers recovery in more depth.

Primobolan for Women

Primobolan is often described as one of the more “female-friendly” anabolic steroids because of its low androgenic rating. That is relative. Its risk of masculinizing effects is lower than that of most steroids, not absent.

Commonly Reported Female Doses

Women most often report 25mg per day of the oral form, or 50-100mg per week injectable, for 4 to 8 weeks. Reports of virilization rise sharply as doses go above that. Women generally do not stack it with other steroids.

Virilization Signs

  • Deepening of the voice
  • Facial or body hair growth
  • Clitoral enlargement
  • Acne
  • Menstrual changes

Voice changes and clitoral enlargement may not reverse after stopping. Counterfeiting is a particular problem for women. In a 2024 study of Australian women who use steroids, participants reported that compounds marketed to women are frequently faked. A product labeled as Primobolan that actually contains a stronger androgen could cause the very effects a woman chose Primo to avoid.

Primobolan vs Anavar

Primobolan and Anavar (oxandrolone) are often compared because both are mild, non-aromatizing steroids used for cutting. They differ in several ways.

PrimobolanAnavar
FormsInjectable or oralOral only
Main useMuscle preservation while cuttingCutting, strength, recomposition
Anabolic rating (commonly cited)88322-630
Androgenic rating (commonly cited)44-5724
AromatizesNoNo
Liver strainLow (injectable); low but documented (oral)Moderate (17-alpha-alkylated)
Hair loss riskHigher (DHT derivative)Lower
Strength gainsMinimalModerate
Virilization risk in womenPresentPresent, usually reported as lower
Counterfeit riskHighVery high

How they differ in practice: Primo is mostly used to hold muscle on a hard cut, and the injectable form avoids the liver. Anavar gives more strength, is oral only, carries more liver strain and is less likely to affect the hairline. Both raise LDL cholesterol and both suppress testosterone. Some users combine Anavar with testosterone instead. See our test and Anavar cycle guide.

Common Primobolan Stacks

These are the combinations most commonly reported. Each extra compound adds its own side effects and, based on the 2023 recovery data, lowers the odds of a full hormonal recovery afterward.

  • Primo + testosterone (replacement-level dose): The most common stack. Testosterone covers the body’s suppressed production while Primo is the main compound.
  • Primo + Anavar: Two mild compounds, popular in contest prep. Both raise LDL cholesterol, so the lipid effect is likely to add up.
  • Primo + Winstrol: A harsher cutting stack. Winstrol is 17-alpha-alkylated and known for joint discomfort and bad effects on cholesterol.
  • Primo + testosterone (higher dose) for a lean bulk: 400-600mg Primo with 400-500mg testosterone is reported for slow, dry gains. The higher testosterone dose brings back estrogen-related side effects.

Legality and Drug Testing

United States: Methenolone is a Schedule III controlled substance. No methenolone product is FDA-approved, so there is no legitimate US prescription route. Possessing it without a prescription is a federal offense, and selling it carries heavier penalties.

Elsewhere: Rules vary. In the UK, anabolic steroids are Class C drugs. Possession for personal use is not an offense, but supply, and importing by post or courier, are illegal. Methenolone acetate remains a prescription medicine in Japan.

Sport: Methenolone is on the WADA Prohibited List at all times, in and out of competition. Anti-doping labs keep finding metabolites that last longer in urine. One 2016 study identified a metabolite that was still detectable 40 days after a single oral dose.

Counterfeit Risk

Primobolan is expensive to make, which makes it one of the steroids most often faked. A 2022 meta-analysis pooled 19 studies covering 5,413 black-market steroid samples. On average, 36% were counterfeit (wrong or missing active ingredient) and another 37% were substandard (wrong dose or contaminated). Cheaper steroids such as drostanolone (Masteron) or testosterone are common substitutes in fake Primo, and their side effects are very different.

The Bottom Line

Primobolan helps hold muscle during a hard cut, but its “mild” reputation oversells the safety and undersells the cost — both financial and physical.

  • It does not aromatize and largely spares the liver in injectable form, but it still shuts down natural testosterone, worsens cholesterol (one clinical case linked it to a heart attack), and roughly half of men in the largest study had not fully recovered their hormones after stopping steroids.
  • It is also one of the most commonly faked compounds on the black market — a 2022 meta-analysis found 36% of underground samples were counterfeit — so there is a real chance what you are injecting is not methenolone at all.

Frequently Asked Questions

Primobolan is often called a “beginner” steroid because it does not aromatize and its androgenic rating is low. That reputation leaves out real risks. It still shuts down natural testosterone, still worsens cholesterol, and is one of the most commonly faked compounds, so a first-time user may not be getting methenolone at all. Its “beginner” reputation also tends to downplay that most reported cycles include a testosterone enanthate base, which brings its own side effects.
Yes. Like all anabolic steroids, methenolone suppresses the body’s own testosterone production. It is often described as less suppressive than nandrolone or trenbolone, but hard comparative data is thin. In a 2023 study, only 48% of men had fully recovered their hormone levels after stopping steroids.
No anabolic steroid is safe for women in the sense of being free of serious risk. Primobolan’s low androgenic rating means virilization is less likely than with most steroids, not impossible. Voice deepening and clitoral enlargement can be permanent. Counterfeit products aimed at women are common, and a fake containing a stronger androgen carries much greater risk.
Users typically report visible changes, mainly lower body fat and harder-looking muscle, after 4-6 weeks, with the full effect by weeks 8-12. These timelines come from user reports, not clinical trials, and depend heavily on diet.
Not in the US without a prescription, and no US prescription product exists. Methenolone is a Schedule III controlled substance. Laws differ elsewhere: personal possession is not an offense in the UK, but supply and importing are. It is banned by WADA at all times.
Oral methenolone acetate is not 17-alpha-alkylated, so its liver risk is usually described as low. But a documented clinical case of liver failure and death after methenolone acetate shows the risk is not zero, especially at high doses or in people with existing health problems. The injectable form avoids first-pass liver metabolism.
Methenolone is costly to produce, and genuine pharmaceutical Primobolan is scarce. The high price is also why it is so often faked: underground labs can sell a cheaper steroid under the Primo label at a large markup.
Men most commonly report 400-600mg per week of the injectable form, with testosterone at roughly replacement-level doses (150-200mg per week). Advanced users report 800-1000mg per week. These are user-reported figures, not clinically tested doses, and higher doses increase cost and side effects.
