Sustanon 250 Cycle: Dosage, Results, Side Effects and How It Differs From Test E
Contents13 sections

Sustanon 250 is a prescription testosterone injection that combines four testosterone esters in one shot. Doctors in the UK and many other countries prescribe it to men with low testosterone. In bodybuilding it is one of the most widely used steroids, usually as the testosterone base of a cycle. A Sustanon 250 cycle is commonly reported as 250 to 500mg a week for 10 to 12 weeks, injected every few days rather than every three weeks as on a prescription.

The blend has a reputation as the “strongest” form of testosterone. It isn’t. Once the esters come off, every injectable testosterone is the same hormone, and milligram for milligram Sustanon delivers about the same amount as testosterone enanthate. What differs is timing: how fast levels rise, how long they last and how long users wait before post-cycle therapy.

This guide covers what is in Sustanon 250, how cycles are commonly run, what controlled trials say about results, the side effects, PCT, and how it compares with Test E. Nothing here is a recommendation to use it. Outside a prescription it is a controlled drug in many countries, and a large share of black-market steroids are not what the label says.

Sustanon 250 is a prescription testosterone medicine. Testosterone is a Schedule III controlled substance in the United States, where Sustanon has never been approved, and it is controlled or prescription-only in most other countries. This article is for educational and harm-reduction purposes and is not medical or legal advice. Anabolic steroids carry serious health risks, including heart disease, blood clots, hormonal disruption and infertility. Sustanon 250 contains peanut (arachis) oil. Speak with a qualified healthcare professional before making any decision about testosterone. Testosterone is prohibited by WADA in all competitive sports. Muscle & Brawn does not encourage the use of anabolic steroids or of prescription drugs obtained outside a legitimate medical relationship.

Key Takeaways

  • Each 1mL of Sustanon 250 holds four testosterone esters: 30mg propionate, 60mg phenylpropionate, 60mg isocaproate and 100mg decanoate. Together they equal about 176mg of testosterone.
  • The blend changes timing, not strength. At the same dose it delivers about the same testosterone as testosterone enanthate, so results at matched doses should be similar.
  • Commonly reported cycle doses are 250-500mg a week for 10-12 weeks, injected every 2-3 days or twice a week. The UK prescribing label gives one 1mL injection about every three weeks.
  • In a controlled trial, men on 600mg of testosterone enanthate a week who also trained gained about 6kg of fat-free mass in 10 weeks. Old claims of 15lb of muscle in 4-5 weeks are not supported.
  • It shuts down natural testosterone and sperm production, thickens the blood and lowers HDL cholesterol. Because of the slow decanoate ester, PCT is commonly started about three weeks after the last injection.

What Is Sustanon 250?

Sustanon 250 is a brand of injectable testosterone originally developed by Organon. It is prescribed for male hypogonadism (low testosterone caused by a problem with the testes or the hormone signals from the brain) in the UK and many other countries, and it is given as a deep injection into a muscle. It has never been approved in the US, where doctors prescribe testosterone cypionate, enanthate and other forms instead.

The “250” is the combined weight of the four esters in 1mL, not the amount of testosterone.

The Four Esters

An ester is a fatty-acid chain attached to the testosterone molecule. It lets the hormone dissolve in oil and slows its release from the injection site. The longer the chain, the slower the release. Once in the blood, the ester is split off and what is left is ordinary testosterone.

EsterAmount per 1mLShare of the BlendRelease
Testosterone propionate30mg12%Fastest: acts within a day or two and clears within days
Testosterone phenylpropionate60mg24%Fast
Testosterone isocaproate60mg24%Medium
Testosterone decanoate100mg40%Slowest: keeps releasing for weeks

Because the esters add weight, 250mg of the blend contains about 176mg of actual testosterone, according to the UK prescribing information. That is about 70% testosterone by weight. Testosterone enanthate is about 72%.

What the Prescribing Information Says

After a single 1mL injection, the UK label reports that total testosterone peaks within 24-48 hours and falls back to the lower end of the normal male range in about 21 days. The usual prescribed dose is one 1mL injection every three weeks, adjusted to how the patient responds.

Two other ingredients matter. The oil is arachis (peanut) oil, so the label says people allergic to peanut or soya should not use it. It also contains benzyl alcohol as a preservative.

What Older Sustanon Guides Got Wrong

Much of what is written about Sustanon 250 goes back to forum lore. Four claims come up most often.

  • “It is the most powerful form of testosterone.” Every testosterone ester delivers the same hormone, and Sustanon contains slightly less testosterone per milligram than enanthate. Differences in results come from dose and injection timing, not from the blend.
  • “Its side effects are like Dianabol’s because the two are similar.” Dianabol (methandrostenolone) is a modified, 17-alpha-methylated oral steroid that strains the liver. Sustanon is plain testosterone given by injection. They share some effects, such as estrogen-related water retention, but they are different drugs with different risks.
  • “Beginners gain 15lb of muscle in 4-5 weeks.” No controlled trial has shown anything close to this. Early weight gain on testosterone is partly water and glycogen. The trial results are covered below.
  • “Legal pills mimic Sustanon without the side effects.” Over-the-counter “legal steroid” supplements do not contain testosterone, and none has been shown to match it. They are also not “FDA approved”: the FDA does not approve dietary supplements before they go on sale.

Sustanon 250 Cycle: Commonly Reported Protocols

The doses below describe how Sustanon 250 is commonly run, based on user reports and coaching literature. They are not clinical doses and they are not recommendations. Prescribed testosterone aims to bring levels back into the normal range. Cycle doses deliberately push far above it.

GoalSustanon 250 DoseOften Combined WithReported Length
First cycle250-500mg/weekNothing else10-12 weeks
Bulking500mg/weekNandrolone decanoate (Deca)12-14 weeks
Cutting250-500mg/weekAnavar or Masteron8-12 weeks
Prescribed TRT (UK label)250mg about every 3 weeksNothing elseOngoing, with blood tests

Injection Frequency

On a prescription, Sustanon is injected every three weeks. Cycle users rarely copy that. The short esters peak within a day or two and fade within days, so a three-week gap means a high peak followed by a long slide. Users commonly report splitting the weekly dose into injections every 2-3 days, or twice a week, to keep levels steadier.

The decanoate ester builds up over the first several weeks, so blood levels keep climbing through the early part of a cycle even at a fixed dose. It also leaves a long tail after the last injection, which is why PCT starts later than with enanthate.

Higher Doses and Stacks

Advanced users report 750mg a week or more, often with other compounds. The best dose-response data on testosterone (covered below) shows that gains rise with the dose, and so do the side effects. Each extra compound in a stack adds its own risks and, according to recent recovery data, lowers the odds of a full hormonal recovery afterwards. Our guide to the best testosterone cycles compares stacks and esters in more depth.

Sustanon 250 vs Testosterone Enanthate (Test E)

Test E is the main alternative to Sustanon and one of the most widely used testosterones in cycles. Both deliver the same hormone. The differences are about timing, availability and convenience.

Sustanon 250Testosterone Enanthate
EstersFour (propionate, phenylpropionate, isocaproate, decanoate)One (enanthate)

How they differ in practice: At the same weekly dose and with frequent injections, a Sustanon cycle and a Test E cycle should produce similar results, because blood testosterone ends up in a similar range. Sustanon’s short esters give a quicker start, which matters little over 12 weeks. Its main drawbacks are the need for more frequent injections to keep levels steady, the longer wait before PCT and the peanut oil. Test E is simpler to run, and in the US it is available on prescription while Sustanon is not. See our testosterone enanthate cycle guide for the single-ester version.

Sustanon 250 Results: What to Expect

Most cycle advice comes from forums, but testosterone is an exception. Two controlled trials tested cycle-level doses. Both used testosterone enanthate rather than Sustanon, but the hormone that reaches the muscle is the same.

What the Trials Found

  • The 1996 New England Journal of Medicine trial (Bhasin and colleagues): 43 healthy men were given 600mg of testosterone enanthate or placebo every week for 10 weeks, with or without weight training. Men on testosterone who trained gained an average of 6.1kg (about 13lb) of fat-free mass. Men on testosterone who did not train gained 3.2kg, and men who trained on placebo gained 1.9kg.
  • The 2001 dose-response trial (Bhasin and colleagues): Men whose own testosterone was switched off with a drug were given 25-600mg a week for 20 weeks. Fat-free mass rose with the dose, by about 3.4kg at 125mg, 5.2kg at 300mg and 7.9kg at 600mg. Higher testosterone levels went with lower fat mass. Hemoglobin rose and HDL cholesterol fell as levels went up.

Fat-free mass includes water and other non-fat tissue, not only muscle. Even so, the best result in the 1996 trial works out to about 0.6kg a week, with supervised training, in screened healthy men on a high dose. That is far from the 15lb in 4-5 weeks promised by older guides.

Commonly Reported Timeline

  • Weeks 1-3: Strength and scale weight start to rise, helped by the fast esters. Much of the early weight is water and glycogen.
  • Weeks 4-8: Lean mass gains become visible, if training and calories support them.
  • Weeks 8-12: Gains slow. Side effects such as acne, water retention or higher blood pressure are more likely to have appeared.
  • After the cycle: Some weight is lost as water drops and natural testosterone recovers. How much muscle is kept depends on training, diet and recovery.

These timelines come from user reports, not trials.

Bulking vs Cutting

On a bulk, testosterone adds lean mass faster than training alone, as the trials show. In a calorie deficit, users mainly run it to hold on to muscle that would otherwise be lost. It is not a fat burner. Fat loss on a cut comes from the diet, even though higher testosterone levels were linked to lower fat mass in the 2001 trial.

Sustanon 250 Side Effects

Sustanon’s side effects are the side effects of testosterone, and they rise with the dose. Prescribed doses carry some of them. Cycle doses carry all of them, more strongly.

Shutdown of Natural Testosterone and Sperm

Injected testosterone tells the pituitary gland to stop releasing LH and FSH, the signals that drive the testes. Natural testosterone production drops, the testes shrink and sperm production falls. The effect on sperm is strong enough that testosterone has been studied as a male contraceptive. Recovery after a cycle is slow and not guaranteed (see PCT below).

Thicker Blood

Testosterone stimulates red blood cell production, and hematocrit (the share of the blood made up of red cells) rises with the dose. Thicker blood is thought to raise the risk of clots. The Endocrine Society’s guideline for men on prescribed testosterone advises pausing treatment if hematocrit goes above 54%. Cycle doses are several times higher than prescribed ones.

Cholesterol, Blood Pressure and the Heart

High-dose testosterone lowers HDL (“good”) cholesterol, as the 2001 trial showed, and users commonly report higher blood pressure during a cycle. Over years, anabolic steroid use is linked to heart muscle damage and faster artery disease. People with existing heart disease or high blood pressure face the most risk.

Estrogen-Related Effects

The aromatase enzyme turns part of the testosterone into estradiol. At cycle doses this can cause water retention, a puffy look and gynecomastia (breast tissue growth). Aromatase inhibitors such as anastrozole are commonly used to control it, but users also report that pushing estradiol too low brings low libido, joint aches and low mood.

DHT-Related Effects

The 5-alpha reductase enzyme turns part of the testosterone into DHT. DHT drives acne and oily skin, speeds up hair loss in men who are genetically prone to it, and can enlarge the prostate.

Mood and Sleep

Responses vary between people. In a 2000 placebo-controlled trial by Pope and colleagues, most men given 600mg of testosterone cypionate a week for six weeks showed minimal psychiatric effects, but a small number developed marked hypomanic symptoms. Irritability is commonly reported. Testosterone can also worsen sleep apnea, which is listed as a risk in its prescribing information.

Injection Risks and Allergy

Sustanon is injected deep into a muscle. Poor technique can hit a nerve or a blood vessel, and non-sterile black-market oil raises the risk of infection and abscesses. Anyone allergic to peanut or soya should not use the original product. Our guide on how to inject testosterone covers sites and technique.

Sustanon 250 PCT

Post-cycle therapy (PCT) is the practice of taking drugs such as tamoxifen (Nolvadex) or clomiphene (Clomid) after a cycle to restart the body’s own testosterone production. Neither drug is approved for this use.

What Users Typically Run

Because the decanoate ester keeps releasing testosterone for weeks, users commonly report starting PCT about three weeks after the last Sustanon injection, compared with about two weeks after enanthate. The most commonly reported protocols are:

PCT DrugCommonly Reported ProtocolDuration
Tamoxifen (Nolvadex)40mg/day weeks 1-2, 20mg/day weeks 3-44 weeks
Clomiphene (Clomid)50mg/day weeks 1-2, 25mg/day weeks 3-44 weeks

Overview
  • Form: Oral Capsules
  • Dosage: 25-40 mg daily for PCT or for gyno flare-ups
  • Typical Cycle: 4-6 weeks
  • Cost: $81.95
  • Stacking Options: Higher testosterone doses or post-cycle recovery
Pros
  • Large mortality benefit in ER-positive breast cancer; five years of adjuvant treatment cuts recurrence roughly by half and breast cancer mortality by about a third, with the effect persisting well past the treatment period
  • Works in premenopausal and postmenopausal women alike, unlike aromatase inhibitors, which makes it the default for younger patients
  • Bone-protective in postmenopausal women, since it acts as a partial estrogen agonist in bone — the reverse of anastrozole's main liability
Cons
  • Endometrial hyperplasia and a two-to-three-fold increase in endometrial cancer risk in postmenopausal women, requiring investigation of any abnormal bleeding
  • Elevated risk of venous thromboembolism and pulmonary embolism, particularly around surgery or immobility
  • Hot flashes, vasomotor symptoms and mood effects are common; CYP2D6 status and strong inhibitors such as certain SSRIs can reduce conversion to the active metabolite

Overview
  • Form: Oral capsules (25 mg each, 60 capsules)
  • Researched Dosage: 25 mg to 50 mg daily for PCT
  • Typical Cycle: 4-6 weeks
  • Cost: $69.95
  • Stacking Options: Tamoxifen, HCG, Enclomiphene
Pros
  • A SERM that blocks estrogen at the hypothalamus, prompting the body to make more LH, FSH and its own testosterone.
  • Studies in men with low testosterone found it raised testosterone while preserving sperm production, unlike TRT.
  • It is oral, inexpensive and has been in medical use since 1967, so its effects are well documented.
Cons
  • About a third of each dose is zuclomiphene, a long-lasting isomer linked to mood swings, irritability and low mood.
  • Blurred vision, floaters and light sensitivity have been reported; visual symptoms are a reason to stop.
  • It is FDA-approved only for female infertility, so use in men is off-label, and this product is sold for research use only.

What the Evidence Says

The best recent data comes from a 2023 study of 641 men seen at a UK clinic after stopping anabolic steroids. Only 48% had fully recovered their reproductive hormones (LH, FSH and testosterone back in the normal range). Men who used PCT recovered faster in the short term, in a median of 13 weeks compared with 26 weeks, but that link only held for men who had stopped within the previous three months. Men who had used more different steroids were less likely to recover. The authors did not endorse PCT and called for proper research.

Blood work before and after a cycle is the only way to know where recovery stands. Our guide to PCT for testosterone covers recovery in more depth.

Sustanon 250 for Women

Sustanon is commonly described as unsuitable for women. Testosterone is strongly androgenic, and the slow decanoate ester keeps levels raised for weeks after the last injection, so the drug cannot be stopped quickly if masculinizing effects start. These include a deeper voice, facial and body hair, clitoral enlargement, acne and menstrual changes. Voice changes and clitoral enlargement may not reverse.

Legality, Drug Testing and Fakes

United States: Testosterone is a Schedule III controlled substance. Sustanon 250 has never been approved by the FDA, so there is no legitimate US prescription for it. Possessing anabolic steroids without a prescription is a federal offense, and selling them carries heavier penalties.

United Kingdom: Sustanon 250 is a licensed prescription medicine. Anabolic steroids are Class C drugs. Possession for personal use is not an offense, but supply, and importing by post or courier, are illegal.

Sport: Testosterone is on the WADA Prohibited List at all times, in and out of competition. Anti-doping labs screen the ratio of testosterone to epitestosterone in urine and use carbon isotope ratio testing, which can tell synthetic testosterone from the body’s own.

Counterfeit Risk

Sustanon is one of the most widely sold underground steroids, and much of it is not the original formula. A 2022 meta-analysis of black-market steroid testing found that, on average, 36% of samples were counterfeit (wrong or missing active ingredient) and another 37% were substandard (wrong dose or contaminated). Underground “Sustanon” may use different ester ratios, a different oil or a different dose than the label claims.

The Bottom Line

Sustanon 250 is ordinary testosterone in a four-ester blend. It is not stronger than Test E; its real differences are timing, injection schedule and a longer wait before PCT.

  • Controlled trials of cycle-level testosterone doses show real but measured gains, about 6kg of fat-free mass in 10 weeks at 600mg a week with training, well short of the 15lb-in-a-month claims of older guides.
  • The risks are predictable and rise with the dose: shutdown of natural testosterone and sperm, thicker blood, lower HDL cholesterol, estrogen and DHT side effects, and a hormonal recovery that only about half of men in the largest study had completed.
  • Much black-market Sustanon is fake or wrongly dosed, and the original product contains peanut oil, so neither the label nor the dose can be taken on trust.

Frequently Asked Questions

No. Both deliver the same hormone. 250mg of Sustanon contains about 176mg of testosterone and 250mg of enanthate about 180mg. At matched weekly doses and with frequent injections, results should be similar. The blend’s reputation comes from its fast early rise, not from extra potency.
The UK prescribing label gives one 1mL injection about every three weeks for men with low testosterone. In cycles, users commonly report injecting every 2-3 days or twice a week, because the short esters clear quickly and a three-week gap causes large swings in blood levels.
Testosterone levels rise quickly: the UK label reports a peak 24-48 hours after an injection. Users commonly report strength and scale weight changes in the first few weeks, with visible lean mass gains from weeks 4-8. Much of the early weight is water and glycogen.
Commonly reported first cycles use 250-500mg a week for 10-12 weeks. Advanced users report 750mg a week or more. These figures come from user practice, not clinical trials, and side effects rise with the dose.
Users commonly report starting about three weeks after the last injection, because the decanoate ester keeps releasing testosterone for weeks. Recovery is not guaranteed: in a 2023 study of 641 men who had stopped steroids, only 48% had fully recovered their hormone levels.
Yes. In the UK and many other countries it is prescribed for men with diagnosed low testosterone, usually as one 1mL injection about every three weeks, with blood tests to adjust the dose. It is not available on prescription in the US, where doctors use testosterone cypionate, enanthate or other forms.
It can. Part of the testosterone is converted to estradiol, and at cycle doses this can cause water retention and breast tissue growth. Aromatase inhibitors are commonly used to control it, but driving estradiol too low causes its own problems, such as low libido and joint aches.
