Anavar (oxandrolone) is often called the “girl steroid” because it is the anabolic steroid women most commonly use. Its androgenic (masculinizing) effect is weak compared with most steroids, so female bodybuilders, physique competitors and gym-goers use it to get leaner and firmer without the higher virilization risk of compounds like testosterone, Winstrol or Dianabol.

Weaker is not the same as safe. Virilization means developing male traits: a deeper voice, facial and body hair, acne and clitoral enlargement. Anavar can cause all of them, especially at higher doses or over longer cycles, and some of these changes can be permanent.

This guide covers what Anavar is, how it works, the doses women commonly report, realistic results and the side effects women need to know about. Nothing here is a recommendation to use it. The FDA withdrew approval of oxandrolone in the US in 2023, and what is sold as Anavar on the black market is often not what the label says.

Anavar (oxandrolone) is a Schedule III controlled substance in the United States, where the FDA withdrew approval of all oxandrolone products in 2023, and it is controlled in many other countries. This article is for educational and harm-reduction purposes and is not medical or legal advice. Anabolic steroids carry serious health risks for women, including permanent voice and genital changes, liver injury, harmful cholesterol changes and menstrual disruption, and they must not be used in pregnancy. Speak with a qualified healthcare professional before making any decision about anabolic steroids. Oxandrolone is prohibited by WADA in all competitive sports. Muscle & Brawn does not encourage the use of anabolic steroids or of prescription drugs obtained outside a legitimate medical relationship.

Key Takeaways

  • Anavar (oxandrolone) is the anabolic steroid women use most, because its androgenic effect is weaker than that of most steroids. Weaker does not mean free of masculinizing effects.
  • Commonly reported female doses are 5-10mg a day for 4-6 weeks. These figures come from user practice, not from trials of physique use.
  • Voice deepening and clitoral enlargement can be permanent. The former US prescribing information told doctors to stop the drug at the first signs of mild virilization.
  • Anavar lowers HDL (“good”) cholesterol, can disrupt periods, suppresses the body’s own hormone production and carries the liver warnings of an oral 17-alpha-alkylated steroid.
  • The FDA withdrew approval of oxandrolone in 2023, so there is no FDA-approved product in the US. Black-market “Anavar” is often fake, underdosed or a different steroid.

What Is Anavar?

Anavar is the original brand name for oxandrolone, an oral anabolic steroid first made in 1962. G.D. Searle & Co brought it to market two years later. It was developed to help patients regain weight and lean muscle, and it was intended to be mild enough for use in men, women and children.

In the US it was approved to help patients regain weight after extensive surgery, chronic infection or severe trauma, to offset the muscle breakdown caused by long-term corticosteroid use, and to relieve bone pain linked to osteoporosis. Doctors also used it off-label, for example in severe burns and in girls with Turner syndrome.

That changed in 2023, when the FDA withdrew approval of all oxandrolone products in the US, including the Oxandrin brand and its generics. The drug’s label had long carried a boxed warning for peliosis hepatis (blood-filled cysts in the liver), liver tumors and harmful changes in blood lipids. There is now no FDA-approved oxandrolone product in the US.

Oxandrolone remains a Schedule III controlled substance in the US, and WADA bans it in and out of competition. Rules in other countries vary. The Anavar used for physique goals comes from the black market.

How Does Anavar Work?

Like other anabolic steroids, oxandrolone binds to the androgen receptor. In muscle, that raises protein synthesis and helps the body hold on to nitrogen. This is why it was used clinically to rebuild lean mass, and why users turn to it during a diet.

Oxandrolone is a derivative of DHT (dihydrotestosterone). The aromatase enzyme does not convert it to estrogen, so it causes no estrogen-driven water retention, and it is not a substrate for 5-alpha reductase. Its commonly cited anabolic rating is 322-630 and its androgenic rating 24; testosterone scores 100 on both.

That low androgenic score is why Anavar is called female-friendly. It is a lab rating, not a guarantee: it lowers the risk of oily skin, acne, unwanted hair and clitoral enlargement rather than removing it.

Oxandrolone is also 17-alpha-alkylated, a chemical change that lets it survive being swallowed but puts more strain on the liver. Its half-life is about 10 hours, which is why users take it every day, often split into two doses.

Benefits

Anavar for women benefits

These are the benefits women commonly report. Most clinical data on oxandrolone comes from men, older adults, burn patients and children, not from healthy women using it for physique goals.

  • Leaner, firmer muscle: moderate gains in muscle tone rather than size
  • Fat loss: mainly when combined with a calorie deficit
  • More strength: often noticeable even without much weight gain
  • Lower virilization risk than most steroids: lower, not zero
  • Higher libido and clitoral sensitivity: some women see this as a side effect rather than a benefit

Muscle Growth and Fat Loss

Oxandrolone increases lean body mass, which is what doctors used it for. In women using it for physique goals, the gains are moderate. Users describe firmer, more defined muscle rather than size, and Anavar on its own does not make women bulky.

It is mainly used while cutting. In a calorie deficit, it helps hold on to muscle that would otherwise be lost. Women who want large amounts of muscle sometimes move to stronger steroids such as Anadrol, which are much more likely to cause virilization.

Because muscle can go up while fat goes down, scale weight may stay flat or even rise during a cycle. Measurements, photos and the fit of clothes say more than the scale does.

Fat Burning

Fat loss is one of the main reasons women use Anavar. In clinical research in older men, oxandrolone reduced abdominal fat, including visceral fat, the fat stored around the internal organs. That is human data, but from men, not women.

Two popular explanations for its fat-burning effect are weaker than they sound:

  • Thyroid hormone (T3): Anavar is often said to raise T3. Androgens lower thyroxine-binding globulin, the protein that carries thyroid hormone in the blood, which changes total thyroid readings. Free thyroid hormone levels usually stay the same.
  • Cortisol and “stubborn fat”: Oxandrolone is anti-catabolic. In burn patients, it helps limit the muscle loss caused by severe stress. There is no good evidence that it lowers cortisol enough to target fat on the hips, thighs or lower belly.

In practice, fat loss on Anavar depends mostly on diet and training. The weekly fat-loss figures promised online are not based on studies.

Enhanced Athletic Performance

Women commonly report a clear rise in strength on Anavar, even without much weight gain. Users also describe fuller-looking muscles and better endurance in training. It is banned in tested sport, so this applies to training, not competition.

Anavar does not raise testosterone. It acts on the androgen receptor directly, and it lowers the body’s own production. Other DHT-derived steroids, such as Anadrol and Proviron, share its reputation for strength. Claims that Anavar works “like creatine” by boosting ATP in muscle have little evidence behind them.

Is Anavar Female Friendly?

Anavar’s reputation rests on its low androgenic rating, and it is less likely to cause virilization than most anabolic steroids. But virilization has been documented at clinical doses. In studies of girls with Turner syndrome given oxandrolone to support growth, voice deepening and clitoral enlargement were among the side effects reported.

The risk rises with dose and cycle length, and sensitivity varies from woman to woman, so no dose is known to be safe. Signs tend to build gradually, which makes them easy to miss at first.

Not all of these changes go away. Acne, oily skin and menstrual changes usually settle after stopping. Voice deepening and clitoral enlargement can be permanent, and new facial or body hair may persist.

The former US prescribing information told doctors to stop the drug as soon as mild virilization appeared, to prevent irreversible changes. Lowering the dose and carrying on, as some guides suggest, does not undo voice or clitoral changes that have already started.

Anavar Dosage for Women

There is no approved dose of Anavar for physique use, and no trial has tested it in healthy women for that purpose. The figures below are commonly reported doses from user practice. They are not recommendations.

Reported Daily DoseHow Users Describe ItReported Cycle Length
2.5-5mg/dayCautious first cycle4-6 weeks
5-10mg/dayMost commonly reported range4-6 weeks
Above 10mg/dayVirilization reports become much more commonVaries

For context, the former US prescribing information gave an adult range of 2.5-20mg a day, usually for 2-4 weeks at a time, for medical weight gain. That range was for patients with medical conditions under a doctor’s care, not for healthy women trying to get lean.

Users commonly split the daily dose into a morning and an evening dose because of the short half-life. Short cycles are used to limit virilization, but a short cycle lowers the risk rather than removing it.

Anavar Side Effects in Women

anavar for women side effects and health risks

Anavar causes milder side effects in women than most steroids, which is where the “girl steroid” nickname comes from. Milder is not the same as safe. The main risks are:

  • Virilization: voice deepening, facial and body hair, acne, scalp hair thinning and clitoral enlargement. Some of these can be permanent (see above).
  • Menstrual changes: late, missed or irregular periods.
  • Suppressed natural hormones: lower libido, mood and well-being are commonly reported after a cycle.
  • Cholesterol changes: lower HDL and sometimes higher LDL, which raise heart disease risk over time.
  • Liver strain: as an oral 17-alpha-alkylated steroid, it carries liver warnings, especially with alcohol or other medicines.
  • Counterfeit products: fake or mislabeled “Anavar” may contain a stronger androgen.

Anavar must not be used during pregnancy. Androgens can masculinize a female fetus.

Libido and Clitoral Sensitivity

Many women report a higher sex drive and increased clitoral sensitivity on Anavar. Anavar is an androgen, and androgens can raise libido in women, so this is expected. Some women welcome it; others find it unwanted.

Clitoral enlargement is a different matter. It is a sign of virilization, it can happen at doses described as low, and it may not reverse after stopping.

Lower Natural Testosterone

Like other anabolic steroids, Anavar can suppress the brain’s signals to the ovaries (LH and FSH). That can lower a woman’s own testosterone production and is one reason periods change. Women make far less testosterone than men, but it still matters for libido, mood and energy.

Some women report lower well-being and libido after a cycle. Hormonal recovery in women has hardly been studied, and there is no tested post-cycle protocol for women. Drugs men use for this, such as tamoxifen and clomiphene, act on the female reproductive system and carry their own risks, including blood clots with tamoxifen.

Higher Cholesterol

Oxandrolone lowers HDL (“good”) cholesterol, and LDL (“bad”) cholesterol can rise, as clinical research has shown. Most of this data comes from men, and the drug’s label carried a boxed warning about lipid changes linked to hardening of the arteries. Women are not protected from this simply because they make less testosterone.

Lipid changes from anabolic steroids usually improve after stopping, but repeated cycles add up. Cholesterol and blood pressure are separate risks, and both are worth checking. Regular cardio helps with both.

Liver Strain

Anavar is a 17-alpha-alkylated oral steroid, so the liver carries more of the load than with injectables. It is often described as milder on the liver than other orals such as Anadrol or Winstrol. Even so, its label carried a boxed warning for peliosis hepatis and liver tumors, and raised liver enzymes are also reported. Mild enzyme rises usually settle after stopping; the rarer serious problems may not.

Women with any history of liver disease, or who drink heavily, are generally advised to avoid it. Oxandrolone also strongly increases the effect of the blood thinner warfarin, which can lead to bleeding.

Monitoring Health During an Anavar Cycle

Blood work is the only way to see what a cycle is doing inside the body. The checks most often used to monitor anabolic steroid use, before, during and after a cycle, are:

  • Lipid panel: HDL, LDL and triglycerides
  • Liver enzymes: ALT and AST
  • Blood pressure
  • Reproductive hormones: LH, FSH, estradiol and testosterone, especially if periods change

Commonly Faked

Anavar is one of the most commonly faked steroids. A 2022 meta-analysis of black-market steroid testing found that about a third of samples were counterfeit, and a similar share again were underdosed or contaminated. Since the US withdrawal, there is no legitimate US pharmacy supply at all.

This matters more for women than for men. A commonly reported problem is cheaper, more androgenic steroids such as Dianabol being sold as Anavar, which raises the risk of the very changes a woman chose Anavar to avoid. Home reagent test kits can suggest which type of steroid a tablet contains, but they cannot confirm the dose, and results in the mirror are not a reliable guide either.

Don’t Use Grapefruit Juice to Boost Anavar

Some users take Anavar with grapefruit juice to try to make a smaller dose go further. Grapefruit juice blocks CYP3A4, an enzyme in the gut and liver that breaks down many drugs, and it can sharply raise blood levels of some oral medicines.

There is no good evidence on how much, if at all, it changes oxandrolone levels. Using it to stretch a dose means not knowing the dose actually being taken, which adds to the virilization and liver risk. Grapefruit also interacts with many prescription drugs.

Why Do Women Take Winstrol Too?

Some women stack Anavar with Winstrol (stanozolol), or move on to it after an Anavar cycle, for a drier, harder look and more fat loss. Our guide to Winstrol for women covers it in detail.

For most women, this sharply raises the risk. Winstrol is generally reported to cause virilization more readily than Anavar, including voice changes that can be permanent. It is also a 17-alpha-alkylated oral, so stacking the two doubles up on liver strain, and Winstrol is known for harsh effects on cholesterol and for joint pain.

Every added compound brings its own side effects. Our Anavar vs Winstrol comparison sets out how the two differ.

Can Anavar Stop Your Period?

Yes, it can. Periods can become late or irregular, or stop, on Anavar, even at low doses, because it interferes with the hormones that control the menstrual cycle. Anabolic steroids can also bring on early or unexpected bleeding.

Cycles often return to normal after stopping, but not always quickly. A missed period can also mean pregnancy, and Anavar must not be used in pregnancy. Changes that last are a reason to see a doctor.

How Long Will It Take to See Results?

Anavar has a half-life of about 10 hours, so blood levels stabilize within a few days. Women commonly report visible changes in the first 2-3 weeks: leaner, with more muscle tone. Reported results tend to level off around weeks 5-6.

These timelines come from user reports, not trials. Diet matters most: results depend on eating in a calorie deficit and training consistently. Longer cycles mainly add virilization risk.

Anavar Female Before and After Results

anavar for women before and after

Before and after photos like this show what some users report, not what is typical. Results depend on diet, training, starting body fat, dose and whether the product was genuine oxandrolone. Lighting, a pump, tanning and posing also change how a physique looks in photos.

The Bottom Line

Anavar is milder for women than most anabolic steroids, but “mild” still includes changes that may never reverse.

  • Women commonly report leaner, firmer muscle and more strength at 5-10mg a day, with most of the visible change in the first 4-6 weeks. These are user reports, not trial results.
  • Voice deepening and clitoral enlargement can be permanent, and stopping at the first signs is the only way to limit them. Cholesterol, liver and menstrual effects need blood work and medical follow-up.
  • With no FDA-approved product left in the US and a black market full of fakes, many women taking “Anavar” may not be taking oxandrolone at all.

Frequently Asked Questions

Women use Anavar mainly to get leaner and firmer. It helps hold on to or add lean muscle, and users report fat loss and more strength, mostly while dieting. Gains are moderate rather than bulky. The trade-offs include virilization (some of which can be permanent), lower HDL cholesterol, menstrual changes and liver strain.
Commonly reported doses are 5-10mg a day, often split into two doses, for 4-6 weeks. These figures come from user practice, not from clinical trials of physique use, and they are not a recommendation. Virilization reports become much more common above 10mg a day, and no dose is known to be free of risk.
Not legally in the US. The FDA withdrew approval of oxandrolone in 2023, and it remains a Schedule III controlled substance. What is sold online or in gyms is black-market product, and a large share of black-market steroids are fake, underdosed or contain a different steroid. For women, a fake that contains a stronger androgen can mean permanent virilization.
Oxandrolone binds to the androgen receptor, which raises protein synthesis and helps muscle hold on to nitrogen. It is a DHT derivative, so it does not convert to estrogen. Its androgenic effect is weaker than that of most steroids, which lowers, but does not remove, the risk of masculinizing side effects.
Users commonly report visible changes within the first 2-3 weeks, with results leveling off around weeks 5-6. These timelines come from user reports, not trials, and depend mostly on diet and training.
Some can be. Acne, oily skin, menstrual changes and cholesterol changes usually improve after stopping. Voice deepening and clitoral enlargement can be permanent, and facial or body hair may persist. The former US prescribing information told doctors to stop the drug at the first sign of mild virilization to prevent irreversible changes.
No anabolic steroid is safe in the sense of being free of serious risk. Anavar is less likely to cause virilization than most, which is why it is called the “girl steroid,” but it can still cause permanent changes, lower HDL cholesterol, strain the liver and disrupt periods, and it must not be used in pregnancy. Our guide to the safest steroids shows how it compares.
