Search for ED treatment and you get a menu: sildenafil, tadalafil, a daily tablet, and now both drugs in one. The menu skips where a clinician starts.
Sildenafil vs tadalafil is mostly about how you want to use the medicine. Both are PDE5 inhibitors, both have been FDA-approved for erectile dysfunction (ED) for over 20 years, and a 2017 review of 16 head-to-head trials found they worked about equally well. A tablet combining the two, like Yes Clinic’s Duo, is compounded and isn’t FDA-approved, and both FDA labels say combinations haven’t been studied.
Before any sildenafil vs tadalafil decision, though, a clinician asks whether you take nitrates or use poppers. If you do, none of the three is for you.

By the numbers:
- 1998: first FDA approval of sildenafil
- 2003: first FDA approval of tadalafil
- 16: head-to-head trials pooled in a 2017 meta-analysis
- 2: FDA labels that say PDE5 combinations haven’t been studied
Sources: FDA labels on DailyMed; Gong et al., International Urology and Nephrology, 2017.
Key Takeaways
- Sildenafil · from $89 for 30, plus shipping: as needed, before sex. The longest track record.
- Tadalafil · from $99 for 30, plus shipping: as needed, or a low daily dose ($109 a month).
- Duo · $199 for 30, about $6.63 a tablet: both drugs in one dissolving tablet. Compounded, not FDA-approved, little research on the combination.
Sildenafil vs tadalafil: same drug class, different instructions
Sildenafil and tadalafil block an enzyme called PDE5, which lets the blood vessels in the penis relax and fill during arousal. Both labels say the drug has no effect without sexual stimulation.
| Sildenafil | Tadalafil | Duo | |
|---|---|---|---|
| What it is | One PDE5 inhibitor | One PDE5 inhibitor | Sildenafil 20 mg plus tadalafil 7 mg, compounded |
| First FDA approval | 1998 | 2003 | Not FDA-approved |
| Label directions | As needed, 30 minutes to 4 hours before sex | As needed before sex, or once daily | Your clinician’s directions |
| Common side effects | Headache, flushing, indigestion, vision changes | Headache, indigestion, back pain, muscle aches | Both lists apply |
| Yes Clinic price | From $89 for 30 (about $2.97 a tablet), plus shipping | From $99 for 30 ($3.30 a tablet), plus shipping | $199 for 30 (about $6.63 a tablet) |
Don’t compare the milligrams, and don’t ask which is “stronger.” The labels use different ranges (sildenafil 25 to 100 mg, as-needed tadalafil 5 to 20 mg), like comparing one guy’s bench in kilos with another’s in pounds.
What 16 head-to-head trials found
The sildenafil vs tadalafil research is best summed up by a 2017 meta-analysis in International Urology and Nephrology by Gong and colleagues: 16 trials, 5,189 patients.
The two came out with similar effectiveness and overall side-effect rates. Men and their partners preferred tadalafil. On side effects, tadalafil vs sildenafil split like this: more muscle aches and back pain on tadalafil, less flushing.
The authors say blinding wasn’t described in detail and that publication bias is possible for some side-effect measures. These trials used the approved drugs, so they say nothing about a compounded tablet like Duo.
So the sildenafil vs tadalafil choice comes down to how you like to use it and which side effects bother you less, worked out with a clinician who knows your history.
Sildenafil vs tadalafil timing: what each FDA label says
The sildenafil label puts the usual dose at 50 mg “approximately 1 hour before sexual activity,” and allows “anywhere from 30 minutes to 4 hours” before. Once a day at most, food or no food. Half-life: about 4 hours.
The tadalafil label starts as-needed use at 10 mg before sex, once a day at most, and says it “was shown to improve erectile function compared to placebo up to 36 hours following dosing.” Average half-life: 17.5 hours.
These numbers belong to the approved products They come from the brand-name drugs’ labels and trials. Compounded tablets, Duo included, haven’t been through those trials. Nobody can promise how fast or how long any tablet will work for you.
If sex is usually planned, sildenafil may suit you; if not, ask about tadalafil.
Daily tadalafil vs taking it as needed
Daily tadalafil is the one option built around a routine: 2.5 mg, raised to 5 mg if needed, “taken at approximately the same time every day, without regard to timing of sexual activity.” It tends to come up for men who have sex often or hate timing a tablet.
The trade-off is in the label. A daily dose keeps tadalafil in your blood all the time, so prescribers are told to look harder at nitrates, alpha-blockers, blood pressure medicines, drugs that raise tadalafil levels, and heavy drinking. If any apply, expect more questions.
Taking sildenafil and tadalafil together: what Duo is, and what the labels say
Duo sits outside the usual sildenafil vs tadalafil choice: it’s a compounded tablet with 20 mg of sildenafil and 7 mg of tadalafil that dissolves on your tongue, prepared by a licensed US pharmacy in your name from a clinician’s prescription. Compounded medications are not FDA-approved, and FDA does not review them for safety, effectiveness or quality.
Read this before you buy any sildenafil tadalafil combination, ours included. The sildenafil label says combining it with other PDE5 inhibitors has “not been studied,” and adds: “Such combinations may further lower blood pressure. Therefore, the use of such combinations is not recommended.” The tadalafil label also says combinations “have not been studied” and tells doctors to “inform patients not to take” it with other PDE5 inhibitors.
We found two randomized trials on ClinicalTrials.gov, both at Egyptian universities and neither with results; the larger, a recruiting 500-man trial in men whose first ED tablet didn’t work, has been recruiting since December 2025. Neither trial tests Duo’s amounts.
A clinician can still prescribe one if, after reading your intake, they judge it appropriate. Lower milligrams don’t mean lower risk: how this tablet is absorbed has not been studied, and no label warning is reduced, nitrates first.
- Never take Duo with another ED medication unless your clinician tells you to.
- Never build your own combo from separate sildenafil and tadalafil tablets. Both labels warn against exactly that.
How quickly this particular formula works has not been established.
What a clinician checks before choosing any of the three
A clinician’s first job is ruling things out. From both labels and the American Urological Association’s 2018 ED guideline:
- Nitrates, poppers and riociguat. Nitrates in any form, taken “regularly and/or intermittently,” rule out both drugs: nitroglycerin, isosorbide and “poppers” (amyl or butyl nitrite), and even occasional use counts. With a PDE5 inhibitor they can drop blood pressure to dangerous levels. Riociguat, a lung blood pressure drug, is ruled out too.
- Your heart. Each label says ED drugs shouldn’t be used by men for whom sex is inadvisable because of their heart. The AUA guideline says men should be told ED can be a risk marker for heart disease.
- Alpha-blockers and other blood pressure medicines, which with a PDE5 inhibitor can cause dizziness or fainting.
- Drugs that raise PDE5 inhibitor levels, such as ritonavir, ketoconazole or erythromycin. The sildenafil label suggests a lower dose with these.
- Alcohol. The tadalafil label flags 5 or more units: dizziness, a faster heart rate, a blood pressure drop when you stand.
- The rest of your history: past sudden vision loss (NAION), liver or kidney disease, and conditions that raise the risk of a prolonged erection, such as sickle cell anemia, leukemia, multiple myeloma or Peyronie’s disease.
- Testosterone. The AUA guideline recommends a morning total testosterone test for men with ED. Our ED plans don’t include one; ask your own doctor. Our guide to TRT costs and the labs that come first covers that route; M&B’s free testosterone range guide explains the numbers.
Tell the clinician everything you take, supplements and gym extras included. If it’s not on the form, nobody checked it. FDA keeps finding hidden ED drugs in “natural” enhancement products (what FDA keeps finding in gas station sex pills), so you could double a dose without knowing it.
Which one fits you
One way to think about sildenafil vs tadalafil vs Duo:
- Sex is usually planned and you want the longest track record: ask about sildenafil first.
- You’d rather not time a tablet closely: ask about as-needed tadalafil.
- You have sex often and like routines: daily tadalafil.
- You and your clinician want a sildenafil tadalafil combination, and you accept a compounded product with limited research behind it: Duo.
If one doesn’t suit you, your clinician can switch you and say how long to wait.
For Duo:
Good fit if you:
- Want both drugs in one dissolving tablet, after a clinician reads your history
- Want to see the per-tablet price first
Skip it if you:
- Take nitrates in any form, use poppers, or take riociguat (rules out all three)
- Have been told sex isn’t advisable for your heart
- Want an FDA-approved product. Duo is compounded.

How Yes Clinic works, and the gaps
Yes Clinic is a telehealth company, not a pharmacy, and it doesn’t make medication. It’s LegitScript certified; LegitScript is a private company, not a government agency, and Yes Clinic pays it for the review (check the listing).
- Fill in a secure health intake.
- A licensed clinician decides whether a prescription is appropriate, and which one.
- If prescribed, a licensed US pharmacy fills and ships it in your name; its name is on your label.
The gaps: there’s no phone line. The care team answers through the patient portal and support@yesclinic.com handles the rest, but that inbox isn’t watched around the clock and health details shouldn’t go by email. Availability varies by state.

The Bottom Line
Sildenafil and tadalafil worked about equally well in 16 head-to-head trials, so the choice comes down to how you want to use the medicine and which side effects bother you less.
- Nitrates, poppers and riociguat rule out all three options, so that’s the first thing a clinician checks.
- Tadalafil lasts longer by its label and comes in a daily dose; sildenafil has the longest track record.
- Duo puts both drugs in one dissolving tablet, but it’s compounded, not FDA-approved, and both FDA labels say the combination hasn’t been studied.
Frequently Asked Questions
Important safety information. Never take sildenafil, tadalafil or Duo with nitrates (such as nitroglycerin or isosorbide) or “poppers” (amyl or butyl nitrite): together they can cause a sudden, dangerous drop in blood pressure. Do not take them with riociguat, or if a doctor has said sex is inadvisable for your heart. Tell your clinician if you take alpha-blockers, blood pressure medicines, or drugs such as ritonavir, ketoconazole or erythromycin; if you drink heavily; and if you have heart, liver or kidney disease, past sudden vision loss, or a condition such as sickle cell anemia, leukemia, multiple myeloma or Peyronie’s disease. Don’t take Duo with another ED medication or combine separate sildenafil and tadalafil tablets. An erection lasting more than 4 hours needs emergency care. Stop and get medical help right away for sudden vision loss in one or both eyes, or a sudden decrease or loss of hearing. Common side effects include headache, flushing, indigestion, nasal congestion, back pain and muscle aches; sildenafil can also cause vision changes, nausea, dizziness and rash. These come from the approved tablets’ labels. Duo has not been studied, so its own side effects aren’t established. In an emergency, call 911.
