HCG for Bodybuilding: Uses, Dosing Research and Side Effects

HCG is a common compound in TRT stacks, as well as in the PCT of bodybuilders. How do you even use it, and should you even stack it with other compounds?

Bodybuilders use HCG (human chorionic gonadotropin) to keep their testes working while testosterone or other steroids shut natural production down. HCG acts like luteinizing hormone (LH), so it keeps the testes making testosterone and sperm even when the brain has stopped sending the signal.

Ask a bodybuilder about HCG and you might get a shrug. Ask a man on testosterone replacement therapy (TRT) who wants kids, and you’ll get a very different answer. That split is the whole story of HCG for bodybuilding: it won’t build muscle by itself, but it protects the things a cycle can take away, namely testicular size, fertility and an easier recovery afterward.

This guide covers how HCG works, what clinical studies show about dosing, the protocols bodybuilders commonly report, the side effects, and how HCG compares with TRT and SERMs.

HCG is a prescription medicine. This article is for educational purposes only and is not medical advice. Doses below are reported from clinical studies and bodybuilding practice, not recommendations. Talk to a doctor before using HCG or any hormone.

Key Takeaways

  • HCG mimics luteinizing hormone, so it keeps the testes producing testosterone and sperm while steroids or TRT suppress the body’s own signal.
  • In men taking 200 mg of testosterone enanthate a week, 500 IU of HCG every other day kept testicular testosterone 26% above baseline, while placebo let it fall by 94%.
  • Men on TRT who added 500 IU of HCG every other day kept normal semen quality for over a year in a 26-man study, and none became azoospermic.
  • In a 49-man case series, an HCG-based combination restored sperm production in 95.9% of men who had lost it on testosterone, after 4.6 months on average.
  • HCG raises estradiol along with testosterone, and it is a prescription-only medicine in the US.

What Is HCG?

Human chorionic gonadotropin (HCG) is a hormone the body makes during pregnancy. Pharmaceutical HCG, sold under brand names such as Pregnyl and Novarel, is prescribed for fertility treatment and for some forms of male hypogonadism.

HCG is made of two parts, an alpha and a beta subunit. Its alpha subunit is identical to that of LH, and its beta subunit is similar enough that HCG binds to the same receptor on the testes. The difference is staying power. LH clears from the blood within minutes to hours, while HCG stays active for a day or more. That’s why a few injections a week can do the job that LH does with constant pulses.

How HCG Works in the Body

Testosterone production runs on a feedback loop called the hypothalamic-pituitary-gonadal (HPG) axis:

  1. The hypothalamus releases gonadotropin-releasing hormone (GnRH).
  2. GnRH tells the pituitary gland to release LH and follicle-stimulating hormone (FSH).
  3. LH tells the Leydig cells in the testes to make testosterone. FSH supports sperm production.
  4. When testosterone is high, the brain turns the whole chain down.
Diagram of the HPG axis showing GnRH from the hypothalamus, LH and FSH from the pituitary, and testosterone feedback from the testes

Step four is where a cycle causes trouble. Testosterone and other anabolic steroids push hormone levels high, so the brain cuts LH and FSH almost to zero. Without LH, the Leydig cells go quiet, testosterone inside the testes drops sharply, sperm production stalls and the testes shrink.

HCG skips the brain entirely. It binds to the LH receptor on the Leydig cells and keeps them working no matter what the rest of the axis is doing. It does not replace FSH, which is why some fertility protocols add HMG (human menopausal gonadotropin) or recombinant FSH.

Why Bodybuilders Use HCG

HCG for bodybuilding comes down to four jobs:

  • Preventing testicular atrophy during a cycle: Keeping the Leydig cells active stops the shrinkage that most steroid users notice within weeks.
  • Preserving fertility: Testosterone in the testes needs to stay far higher than in the blood for normal sperm production. HCG keeps it there.
  • Easing post-cycle recovery: Testes that were kept active through a cycle don’t have to be “woken up” from scratch when the cycle ends. Many users run HCG before starting post cycle therapy (PCT).
  • Supporting TRT: Many men on testosterone therapy take HCG alongside it so they keep their fertility and testicular size.

The longer and heavier the suppression, the more these jobs matter. A 2014 review in Fertility and Sterility on steroid-induced hypogonadism notes that low testosterone after steroid use can depend on the dose, duration and type of compounds used.

HCG Dosage for Bodybuilding: What the Research Shows

Most of the solid data on HCG dosing comes from clinical studies of men on testosterone, not from bodybuilders running multi-compound cycles. Here is what those studies used and found.

StudyWhoHCG DoseResult
Coviello et al., 200529 healthy men on 200 mg testosterone enanthate weekly125, 250 or 500 IU every other day for 3 weeksTesticular testosterone fell 94% on placebo. It was 25% below baseline at 125 IU, 7% below at 250 IU and 26% above at 500 IU
Hsieh et al., 201326 hypogonadal men on TRT500 IU every other daySemen quality held steady for over a year; no man became azoospermic and 9 of 26 men contributed to a pregnancy
Madhusoodanan et al., 201920 men with low-T symptoms and testosterone above 300 ng/dLHCG aloneTestosterone rose 49.9%, from 362 to 520 ng/dL on average; half reported fewer symptoms
Wenker et al., 201549 men who lost sperm production on testosterone3,000 IU every other day plus other fertility drugsSperm production returned in 95.9% of men, after 4.6 months on average

Commonly Reported Bodybuilding Protocols

Outside of studies, these are the HCG protocols bodybuilders most often report. They are community practice, not tested regimens, and they vary widely:

UseCommonly Reported DoseNotes
During a cycle250 to 500 IU, 2 to 3 times a weekClose to the 250 to 500 IU every-other-day range used in the Coviello study
Alongside TRT250 to 500 IU, 2 to 3 times a weekOften prescribed by TRT clinics to protect fertility
Before PCT (“bridge”)1,000 to 2,000 IU every other day for 1 to 3 weeksUsed to restart the testes before SERMs; little direct research
Fertility recovery1,500 to 3,000 IU, 2 to 3 times a week for monthsUsually doctor-supervised, often with HMG or FSH

More is not better. Users and clinicians commonly warn that long stretches of very high doses can blunt the testes’ response to LH, and higher doses also push estradiol up faster.

HCG comes as a powder that has to be mixed with bacteriostatic water before use. Our peptide reconstitution calculator works out the volume per dose, and our guide to peptide injection sites covers subcutaneous injections.

The Fertility Recovery Protocol (HCG, HMG and L-Carnitine)

Bodybuilders who have spent years on testosterone often hear about a “baby-making protocol”:

  • HCG: 1,000 to 3,000 IU on Monday, Wednesday and Friday
  • HMG: 75 IU on Tuesday, Thursday and Saturday
  • Injectable L-carnitine: 500 mg or more daily

No trial has tested this exact stack. The closest clinical evidence is the Wenker case series above, where doctors combined HCG with other fertility drugs and most men recovered. Recovery isn’t guaranteed, though. A 2017 study found that older men and men who had used testosterone longer took longer to recover, and only 70% reached a total motile sperm count above 5 million. Anyone trying to conceive after long-term use should work with a urologist and track progress with semen analyses.

HCG Benefits for Bodybuilders

Testicular Size and Function

HCG keeps the Leydig cells active, so the testes keep their size and their testosterone output during a cycle. This is the benefit most bodybuilders notice first.

Fertility

This is where the evidence is strongest. In the Hsieh study, men on TRT plus HCG kept normal semen quality for more than a year, and several fathered children during the study. Without HCG, testosterone therapy can lead to zero sperm in the ejaculate in a large share of men.

Testosterone and Symptoms

In men whose own testes still work, HCG alone can raise testosterone. The 2019 Madhusoodanan study saw an average rise of about 50%, and half of the men felt better. Those levels stay in the normal range, so HCG on its own won’t produce the gains of a steroid cycle.

Sexual Function

Users commonly report better libido, fuller-feeling testes and normal ejaculate volume when they add HCG to TRT or a cycle. These reports are consistent, but they come mostly from anecdotes rather than trials.

HCG Side Effects and Risks

  • Higher estradiol: HCG raises testosterone inside the testes, and some of it converts to estradiol. High estradiol can cause water retention, nipple sensitivity and, in some men, gynecomastia. Blood work is the only reliable way to check.
  • Acne and oily skin: Commonly reported, especially at higher doses.
  • Injection site reactions: Mild redness, bruising or soreness.
  • Mood changes and headaches: Reported by some users.
  • Reduced response at high doses: Long runs at very high doses may make the testes less responsive to LH, which defeats the purpose.
  • Not a weight-loss drug: The FDA-approved labeling for HCG states it has not been shown to be effective for treating obesity, despite the “HCG diet” products sold online.

HCG vs TRT

HCG vs TRT infographic comparing how each affects testosterone, LH and fertility

HCG and TRT both raise testosterone, but they do it in opposite ways. TRT replaces your testosterone from outside, which tells the brain to stop sending LH. HCG stands in for LH, so your own testes keep doing the work.

HCGTRT
How it worksStimulates the testes to make testosteroneSupplies testosterone from outside

Testicular sizeMaintainedOften shrinks
Testosterone levelsUsually mid-normal rangeNormal or higher, depending on dose
Best fitFertility, cycle support, mild low TClearly low testosterone, symptom relief

For many men it isn’t either-or. TRT clinics often prescribe HCG alongside testosterone enanthate or cypionate so patients get stable testosterone levels and keep their fertility. If you’re comparing providers, our roundup of the best online TRT clinics compares the main options. Ask any clinic up front whether HCG is part of its protocol.

Between cycles, the choice depends on your plans. Many bodybuilders with another cycle a few weeks away “cruise” on a low testosterone dose instead of coming off. Those who want to recover fully, or who want children, usually run HCG and then a SERM-based PCT.

HCG vs SERMs

SERMs (selective estrogen receptor modulators) such as Nolvadex, Clomid and enclomiphene also raise testosterone, but they work on the brain instead of the testes.

HCGSERMs
Where it actsDirectly on the testesOn the hypothalamus and pituitary

Works while on a cycle?YesMostly no, because steroids keep the brain suppressed
Main use in bodybuildingDuring a cycle and as a pre-PCT bridgePCT after the cycle ends

The two aren’t rivals so much as tools for different stages. HCG keeps the testes active while the brain is shut down. SERMs restart the brain’s own signal once the cycle is over. That’s why many PCT plans use both, one after the other. Our guide to the best PCT cycle to keep gains covers the full sequence.

In the US, HCG is a prescription-only medicine. It’s prescribed for fertility treatment and for some men with low testosterone, and doctors commonly add it to TRT. The legal route is a doctor or a licensed TRT clinic.

HCG is also sold online by research chemical sellers, labeled “not for human consumption.” Buying from them means no prescription, no medical oversight and no pharmacy-grade quality control. Laws differ by country and some US states, so check the rules where you live. HCG is also banned in competition for male athletes under the WADA Prohibited List.

Where to Buy HCG

The prescription route through a doctor or TRT clinic is the safest option. The Bottom Line

HCG for bodybuilding is a support tool, not a muscle builder.

  • It keeps the testes working while testosterone shuts the brain’s signal down.
  • Clinical studies support low doses during testosterone use for keeping testicular testosterone and fertility intact.
  • Recovery after long-term use is possible but slower for older men and long-time users.
  • Get blood work, keep doses moderate and involve a doctor if fertility matters to you.

Frequently Asked Questions

Not directly. HCG raises your own testosterone, but usually only into the normal range, so it won’t produce the size or strength gains of a steroid cycle. Its value in bodybuilding is protecting testicular function and fertility, which can make recovery after a cycle easier.
Bodybuilders commonly report 250 to 500 IU two or three times a week during a cycle. That’s close to the doses used in a 2005 clinical study, where 500 IU every other day kept testicular testosterone above baseline in men taking 200 mg of testosterone enanthate a week. These are reported doses, not medical advice.
It usually takes months. In a 49-man case series, sperm production returned after 4.6 months on average with an HCG-based combination. Older men and men who used testosterone for longer tend to take more time, so a urologist and regular semen analyses are worth it.
Yes. HCG increases testosterone inside the testes, and some of it converts to estradiol. Higher estradiol can cause water retention or nipple sensitivity. Blood work is the best way to check your levels before reaching for any estrogen-control drug.
Sometimes. Men whose testes still work can raise testosterone with HCG alone, and one study saw an average rise of about 50%. Men with very low testosterone or failing testes usually need TRT, often with HCG added to protect fertility. A doctor can tell you which fits.
Unmixed HCG powder is stored according to the label, usually at room temperature or in the fridge. Once you mix it with bacteriostatic water, keep it in the fridge and don’t freeze it. Follow the product’s instructions for how long mixed HCG stays usable.
