it doesn't look bad to me, last couple you could see that you were starting to bow your back.. just at the begining of the lifts...

Looks good man. Your hips did come up a hair too early on the last rep but it was trivial - extremely minor. This happens to me too on multiple rep sets. Your back stayed locked during. Awesome job!

same as everyone else...looks solid
when you get to real heavy weights you will want to sink ur A$$ down and drive with those hips, basically when you pull up with the weight pull back also. Use the weight to your advantage, its basically like getting a bit of leverage.

I find nothing terribly wrong with your form. I agree with BTB, your butt came up a bit early on the last rep, but nothing major. I would recommend that you stay the course, and invest in some sweat pants so that you can drag the bar up your shins. Good job.
