Not focusing on improving their strength.
Going from a 95 to a 135 pound barbell row really isn't going to do much. On the other hand, going from a 95 to a 315 barbell row...now you're in business. Keep strong(er) and keep it simple.

Not using the correct exercises, they need to be focussing on the big-boy compound lift. They believe they can get big with cable cross-overs and bicepT curls.
