A few months back, some poster in bodybuilding.com forums mentioned that he broke his arm trying to squat - apparently he was squatting 250ish lbs, and didnt exactly put the bar back in the jay hooks while racking and getting it off his back. Result - 250 lbs of torsional force snapped the forearm.

Unfortunate though this may be, freak accidents do occur in weightlifting (as in any other physical sport).
