You're not consistently eating over maintenance.

Most would say program hopping, I'm going with DOING THE SAME THING EVERY DAMN TIME YOURE IN THE GYM! Forgive the caps, it just pisses me off. People come in and do the same curls, forearm isolation, and shitty form pulldowns everyday of the week, they get nowhere, then they look at me condescendingly for doing low rep power cleans. Either way, add weight to the barbell, and do movements that require a barbell.

Yeah program hopping would be a huge one. Critical thinking is required in order to know what to move around and whether it is really helping you or not.

you are overtraining, which leads to your body releasing cortisol, a muscle wasting hormone, keep workouts short and train intense

Only lifting for 45 minutes, being afraid of a long training session.

Not maximizing every set.

Wasting time between sets and making a short workout take hours.
