Excessive volume and lack of balance are the first two that come to mind, but there are plenty more.

Not focusing on improving their strength.

Going from a 95 to a 135 pound barbell row really isn't going to do much. On the other hand, going from a 95 to a 315 barbell row...now you're in business.

Keep strong(er) and keep it simple.

How about thinking they will get really big really fast? I know a guy who did not know why he was not seeing results in the first few weeks.

Abs are overrated. You can look like this and have abs... Or you can look like this and have pride. I don't care how awesome your abs look when my d*ck can move more weight than you can.

Not using the correct exercises, they need to be focussing on the big-boy compound lift. They believe they can get big with cable cross-overs and bicepT curls.
