I think a lot concerning width of grip rests on the shoulder flexibility or lack thereof for each individual, some of that may boil down to a person with more lean mass being less flexible in their shoulder area or they may have much better ROM in that area than a person with a prior shoulder injury etc...hence so many variations in grip which all ultimately come down to getting correct form where each person is concerned, which is all that matters, along with a stable base/grip itself.

Just for interest: Oly lifters (not squat related) tend to find their best grip width for the chosen discipline and then the coach makes a note of it, so that in cases of comp nerves when they are liable to forget the width they use, they can be given the info and mark the barbell accordingly (with chalk or whatever they do).
