Default Annoying Postural Issues

I have bad posture. My shoulders automatically sit forward, and my shoulder blades sick out of my back. Even when I squeeze my back together like in a bench press my shoulder blades still stick out of my back, my upper traps seem to take over. I have been stretching my pecs, lats and traps, and doing rows and pull-aparts with bands at home, on top of having more pulling than pushing in the gym. I have had no improvements from this. I went to a physio and he gave me some exercises to strengthen my lower traps etc, but nothing has changed. It is impossible for me to hold myself in good posture for long periods of time. I'm getting frustrated. Anyone have any tips to improve posture?

I've had some slight spinal alignment issues dissipate over the course of the 8 months I've been training. I think improving core strength and adding a bit of muscle mass had helped improve my posture, and possibly the focus on posture/ form when lifting has helped maintain a more erect position habitually.

Maybe over more time you'll experience some improvements, however slight.
