I have had that happen benching where one wrist just suddenly folded over. I was back at my old gym and I was lucky I actually had a spotter that day as I didn't always. Bar flew right into the upright bars and was coming down on my face when the guy grabbed it. Scared the crap out of me so I know exactly how you feel, and now I never don't have a safety mechanism, even during warmups.
