IMO, it's usually a combination of things that are awry rather than just 1 thing. However, usually it's diet related. Even something as simple as a Vitamin D deficiency can cause a problem. I would increase calories just a bit at a time, say 100-200 per day, and make sure your getting all the macros and the micros you need every day. You can make adjustments to the macros, like more carbs, one at a time and then re-evaluate.
