I can't tell you anything about your form, I just wanted to wish you good luck. I'm also working on form issues. Do you have anyway to video yourself? That's helped me a lot.
BTW- wouldn't a 5 x 5 program be good for someone trying to put on muscle? That's a lot of time under tension
