I don't know about the nature of his reconstructions, and if there are any lingering issues, but most heavy pulls and leg movements are going to stress the knee to a degree. One thing I recommend is finding out if his squat hesitancy is mental or physical.

And in answer to the question how heavy is too heavy:
"For me its not so much about lift ceilings, if I get to 10 reps then I will move up in weight"
